2011 Kindergarten Round-Up Registration Packet
Be sure to check out our kindergarten website for information on our programming.
The following documents are also required for registration:
- Original birth certificate (with seal) and a copy. - Copy of Immunization records with proof of DTP, Polio, MMR, Hepatitis B, Chicken Pox shots (or proof of having had the illness), and vision screening. - Proof of residency - copy of one of the following: certificate of occupancy, purchase agreement, builders permit, rent receipt or agreement, mortgage statement/payment receipt, voter registration card, utility bill or driver’s license. |
